This form is ONLY for residents of care homes and nursing homes wishing to register as a patient at Braehead Medical Practice.

To start your registration with us, please complete the form below to ensure that we have your relevant personal details. Answer all questions as accurately as possible. If this is your first time registering with a GP practice in the UK, please note that medical records from outwith the UK cannot be traced.

Please note: processing your registration can take up to 14 days.

Please be aware that we only accept new patients who live within the PA4 postcode area. You can only register with Braehead Medical Practice if you’re staying in our area of service for more than 3 months.
